If you want to achieve one or more of these goals then the Health Trainer Service may be for you.
What is the Health Trainer Service?
Health Trainers provide motivation, encouragement and support if you want to make healthy lifestyle changes. They will work with you to identify specific behaviours that you want to change, set achievable goals and find other agencies that can help.
Health Trainers work with people who need a little extra help and support in order to make those changes happen.

Louise takes more than 19,000 steps towards a healthier lifestyle, thanks to NHS Norfolk's Health Trainers
Louise Sanders weighed about 16.5 stone and had a BMI of 34. She had tried to lose weight several times before, but the turning point came after Christmas 2010 when she decided to seek extra support.
Today Louise, who lives in Thetford, is already 20lbs lighter, and is already feeling the physical and psychological benefits of her new lifestyle.
It's all thanks to her working with Emma Rogers, a health trainer at Thetford. From their first meeting they looked at small changes that could be made to Louise's lifestyle that would raise her confidence and help towards her weight loss goal.
Today Louise, 37, has more energy to play with her two young sons, and she cycles and walks more - once clocking up more than 19,000 steps in one day!
"I feel a lot different. Getting in and out of the car is easier, as is running up and down the stairs. Increasing my exercise is helping me lose weight. Life in general is just getting easier," she said.
"It's about changing your mindset. It's being able to recognise that you can have a drink without having something to eat at the same time. I would now choose to walk into town rather than take the car. I don't feel like I want to sit down."
