Personalisation
An introduction to personalisation
Personalisation is an approach to health and social care which puts the person first and their condition second. It recognises people’s own expertise by reinforcing the idea that individuals are best placed to identify their own needs and the most effective ways in which these needs can be met.
Personalisation is not a new idea, but it is a relatively new term. In order to explore what personalisation means in Norfolk and Waveney the SWIFT Team has produced a booklet and short film. We encourage you to take a look at the booklet for further understanding of initiatives taking place in Norfolk and Waveney:
